Forms are a great way to collect information from users on your website; from registration to “contact us”, a form can help you understand and interact with your audience better. To add or edit a Gravity Form on your site, you can contact the Web Team (webmaster@umw.edu) for assistance.

Existing Form Updates
When reaching out about updates to an existing form, there are a few things we need from you:
- the URL for the page where the form exists;
- a description of what fields on the form need to be updated;
- the new content that needs to be added or deleted from those form fields.
If you are looking to add new fields to your form, please let us know what the field should be called, where to place it in the form, and whether or not that field is required.
If you need to add or remove a recipient of form submissions, you can send us the name and email address for those users and we can make those adjustments.
New Form
When reaching out about a new form for your site, please include the following in your request:
- the URL for the page where the form will go (please indicate if it is being added to a new page);
- the names, content, and types for each field in the form;
- indications for which fields are required;
- the names and email addresses for anyone who needs to receive the submissions from the form;
Types of Fields
Gravity Forms has a number of common fields that can help you build the form you need. The following form fields are available to us when creating your form:
- Name
- Phone Number
- Address
- Date
- Time
- Text Field (for free response or other text response needs)
- Radio Buttons (to allow only one option to be selected)
- Checkboxes (to allow multiple options to be selected)
- Drop Down
- File Upload

Form Submissions
In addition to receiving submissions to your UMW email, you can also view form submissions any time in the WordPress dashboard by going to Gravity Forms > Entries and making sure that the dropdown that appears at the top of the window is displaying the correct form name.
Personally Identifiable Information
Gravity Forms cannot collect anything considered “personally identifiable information” for security reasons. That information includes but is not limited to:
- Social Security Numbers
- Credit card information
- Banner ID numbers
- Medical records
