Display Concept Proposal Form

Please fill out the following form for any display ideas that are in the early concept stages of planning and components such as images, text, permissions, stakeholders, funding sources, etc. have not been drafted or identified.

Display Concept Proposal Form

  • Display Proposal Introduction

  • Provide a brief description of the proposed display including the topic, people who will be represented, the placement/location, the display medium, why this topic/people should be represented by a public, long-term or permanent display.
  • Contact Information

  • Please list any additional people or departments involved in the creation of this display proposal.
  • Display Details/Information

    Please provide the following information with as much detail as possible regarding your proposed display.
  • Ex: framed artwork, posters, hanging, other media, etc.
  • The display’s proposed location including the campus, building, or exterior location on campus. Provide the floor or room number if relevant.
  • Provide the approximate dimensions of the display.
  • Will the display be permanent (existing longer than 5 years) or long term (existing more than 1 year but less than 5 years)?
  • Include dates and timeline of when the artwork will be displayed and for how long.
  • Please list potential sources for imagery/artifacts, narrative, research material, etc. (if applicable)
  • Please provide a list of individuals to be represented or depicted in the display who might need to give permission to be included.
  • Please provide background information on which departments, offices, or individuals with interest in the display’s proposed location or content have been contacted and given input or approval for the project. Attach any documentation in the following attachments section, if applicable.
  • Use this section to attach any evidence/documentation that you received permission from the individuals to be depicted in the display, if applicable, and evidence of approval from other University and community stakeholders of the location and content of the proposed display.

    • Please include any and all other attachments needed for the Campus Environment Committee to better understand your display proposal for review. This can include images included in the display, sample research material, additional evidence of required permissions, sketches of the display, etc.
